Our services are available to patients of all ages, including minors. When a patient is a minor, we expect that patient intake forms and website contact forms will be submitted by a parent or legal guardian.

Patient intake information submitted through JotForm is retained in accordance with our clinical record retention obligations under Michigan law and applicable federal regulations, and as described in our HIPAA Notice of Privacy Practices.
Aggregated analytics data collected by Google Analytics is retained in accordance with our configured retention settings for 14 months.

We take reasonable precautions to protect the information collected through this Site. Our contact form and JotForm intake forms are transmitted using encrypted connections (HTTPS). We do not store credit card numbers through this website.
