Completing your paperwork in advance saves time at your first appointment and helps your therapist prepare for your visit. Select the form(s) that apply to your upcoming treatment below. Patient Intake Form
Required for all new patients. Covers your health history, insurance information, and reason for your visit. Please complete this before your first appointment.
Dry Needling Consent
Required before receiving dry needling treatment. Please review and complete this form if your therapist has recommended dry needling as part of your care plan.
Laser Therapy Consent
Required before receiving laser therapy treatment. Please complete this form if laser therapy has been recommended by your physical therapist.
Return to Sport Testing
For athletes preparing for return-to-sport evaluation. Complete this form in advance of your functional testing appointment.
